Abstract: Numerous boiler tube failures have recently occurred in the roof area of the furnace wall in an O-type package boiler at a mid-size oil refinery. Plant BFW treatment is softened with NaZ and failures are believed to have occurred during the shutdown process in preparation of the boiler's annual inspection. The failures have been attributed to steam blanketing and caustic gouging caused primarily by inadequate boiler water circulation. This paper reviews the key variables that contributed to the failures. The critical role of design circulation and water chemistry/treatment will be discussed and cost-effective remedial measures identified.
