51313-02331-Fatal Accident in Dutch Swimming Pool Caused by Environmentally Cracked Bolts

In November 2011 a 5 month old baby unfortunately got killed in a Dutch swimming pool because 2 speakers and a speaker frame landed on her head.  This paper will describe how this accident could happen and what has been done in the Netherlands since 2001 when the entire ceiling and airchannels came down in another swimming pool (luckily this occured during closing time).

In November 2011 a 5 month old baby unfortunately got killed in a Dutch swimming pool because 2 speakers and a speaker frame landed on her head. The speakers fell from 5 meter height because a stainless steel bolt was broken due to environmental cracking (condensation of Chloroform on the stainless steel surfaces). Three inspection reports (2006 2009 and 2011) had indicated immideate replacement of the stainless steel parts by hot dipped galvanised parts or duplex coated parts. Duplex means: galvanised + organic coating. Unfortunately these reports had not lead to the correct action by the swimming pool management and maintenance people of the municipal who is responsible for the safety in this swimming pool. Worldwide many accidents and incidents occurred with cracking stainless steels in the swimming pool atmoshere. In 1985 in Switzerland 12 people got killed by a collapsing roof in a swimming pool.

This paper will describe how this accident could happen and what has been done in the Netherlands since 2001 when the entire ceiling and airchannels came down in another swimming pool (luckily this occured during closing time).

Further a new standard will be discussed: "Materials Selection and Inspection of Fasteners and other Loaded Elements in the Indoor Swimming Pool Atmosphere". This Standard in the Dutch language has been issued on March 2012 by NACE Section Benelux.
